Google Forms is a great app that you already have if you have a Gmail account. With it you can quickly create questionnaires, surveys and quizzes! JC Lenk and Garth Holman of Teachers for Tomorrow in conjunction with the Research Center for Educational Technology at Kent State University, have created an excellent series of video tutorials that take you from a beginner to pro.
